HEALTH OFFICE: The health
office at Alta Loma High school is
staffed with a full time credentialed
School Nurse, who is a licensed Registered
Nurse, and a Health Assistant. Our
Health Assistants are trained, unlicensed
personnel who have CPR and First Aid
training as well as training in the
handling of school emergencies.
MEDICATION ADMINISTRATION:
If it is necessary for your student
to received medication, prescription
or over the counter(such as Advil,
Tylenol, etc), at school it is important
that certain procedures be followed
to ensure the proper administration
of that medication. California Education
Code 49423 requires that all medication,
including over the counter medication,
must come to school:
" In it's original container
(not baggies)
" With a pharmacy label listing
student name, name of drug and dosage
information
" Written permission from the
physician and the parent
Prescriptions must be written by
a California health care provider
licensed to write prescriptions and
filled by a California pharmacy. Consent
forms are available in the Health
Office and on the school web site.
These procedures are required so that
your student receives their medication
exactly as your physician prescribed.
A parent or other adult must bring
the medication to school. Students
are not allowed to bring medications
to school.
EMERGENCY CARDS: In order
to care for your student, California
Education Code 49403 requires that
all parents submit an emergency card
with information as to the parents'
home phone number, work phone number,
and emergency contacts name and phone
numbers. IT IS IMPERATIVE THAT WE
RECEIVE THIS INFORMATION AT THE START
OF EACH SCHOOL YEAR. With out an emergency
card on file if your student is injured
or ill, we may have to call 911 for
treatment at the parent's expense.
Please include at least 2-3 local
numbers of persons who could care
for your student if you are not available.
HEALTH CONCERNS: In order
to ensure your student's health and
safety, it is important that we are
aware of any health condition that
they may have. Please note ANY health
problem in the appropriate space on
the emergency card. This information
is necessary in order to meet any
health and safety concerns for your
student and for any emergency personnel
that may care for your student. This
information is confidential and will
only be shared with those staff that
have a need to know. Please contact
the school if you have any questions.
IMMUNIZATION REQUIREMENTS:
In order for your student to enroll
in school, they must have received
all of their necessary immunizations,
or have a signed personal belief or
medical waiver on file. This is extremely
important to safeguard your child
from diseases. Contact the school
health office for clinic information.
ACCIDENT INSURANCE: As a reminder,
injuries frequently occur with students
and your medical insurance may not
cover all the costs. A low cost accident
insurance policy is available for
parents to purchase to help offset
those costs. Please review your medical
coverage and consider accident insurance
if your family has a need. Application
packets are available at the school.
